Coartem® For New Born and Young Infants: Novartis’s Breakthrough Malaria Medicine

Malaria is one of the deadly disease that has affected the health of many people specifically newborn babies across the globe. The World Health Organization reported that malaria continues to claim immeasurable damages to the life due to lack of specific treatment globally, specifically in countries like Africa. It is important to highlight here that nearly 30 million newborn babies are born are born annually in high malaria risk areas of Africa and infection chances range between 3.4% and 18.4% in infants under six months.

However, the groundbreaking approval of Coartem®, also known as Riamet® Baby by the Swissmedic, Switzerland’s regulatory agency first time in history offer a new ray of hope to address the critical need of treatment. The pharmaceutical giant in collaboration with Medicines for Malaria Venture (MMV) made this announcement on 8 July 2025, indicating a key milestone for public health. This significant achievement has given new direction to pharmaceutical industry to initiate more rigorous research on anti-malaria drugs and malaria diagnostic technique to treat newborn patients.

Malaria: Global Burden and Treatment Gap for Infants

More than three decades, malaria continues to be a burden on healthcare industry. Until now, regulatory agencies, such as the USFDA hasn’t approved malaria treatment for newborn babies or infants weighing less than 4.5 kilograms, creating a significant gap of treatment. Contrastingly, the available malaria treatment have not been trialed / tested on young infants at least 6 months as treatment trials are prohibited on children.

Newborn babies have underdeveloped immune system and immature liver function, so drug formulations specifically malaria vaccines may not be ideal for babies. In addition, malaria parasites can lead to severe illness and even fatal for babies if it remain untreated. These parasites destroy red blood cells that may lead to shortage of oxygen-carrying cells. Moreover, the destruction of red blood cells can cause severe anemia, a condition where there is an extremely low number of cells that carry oxygen. The deep anemia is the main cause of a series of emergencies to be triggered, such as respiratory distress, organ damage (lack of oxygen), cerebral malaria (where the parasites affect the brain), and eventually, multi-organ failure and death.

Given the treatment gap and urgent requirement to address the concern, several pharmaceutical industrial players are continually researching advanced drug formulation for infants. With continuous efforts, in July 2025, Novartis announced that its Coartem® (artemether-lumefantrine) baby received approval from Swissmedic as the first malaria medicine for newborns and young infants.

Coartem® Baby: A Revolutionary Solution

Coartem® Baby emerged as a significant advancement in antimalaria therapy for newborn and young infants. This medication is a reformulated version of artemether-lumefantrine, which marked as a gold standard treatment for malaria in adults and older children. Novartis has designed new formulation by combining different dose ratio of two core ingredients, such as artemether and lumefantrine. These formulations are specifically calibrated for the metabolic differences in babies weighing between 2 and 5 kilograms.

This clinical formulation process involves the combined efforts of Novartis, The Medicines for Malaria Venture (MMV), and European & Developing Countries Clinical Trials Partnership and Swedish International Development Cooperation Agency.

The approval to Coartem® has been provided based on positive results from the Phase II / III CALINA study. This clinical study specifically investigated the new drug formulation ratio and dosing regimen for infants under 5 kilograms.

One of the most innovative aspect about Coartem® is that this medication has been formulated as an infant-friendly formula. It comes as a dissolvable tablet (cherry flavor) so it can quickly be mixed with breast milk for infant administration. This milestone surely addresses the challenges face by healthcare providers when treating malaria in infants.

The Coartem Baby approval tackles an enduring clinical challenge because it enables safe and age-appropriate malaria treatment for infants. This is particularly important because the younger infants have been mostly neglected in treatment trials and policies, which exposes them to blighting consequences of malaria. The possible benefits to child survival and their long term health is immense.

Coartem Baby will strengthen the treatment arsenal against infant mortality and severe malaria in children below 5kg by providing tailored and safe treatment within the stipulated weight range of 2-5kg. During the neonatal stage, the risk of even a small number of parasites triggering severe malaria and potential death stresses the need for timely intervention during this phase.

Even though Novartis have made remarkable innovations with Coartem Baby, its full potential will only be realized with strong multidimensional health coordination on a global level. Sufficiently responsive and flexible solutions are essential to cope with the unremitting vicious cycles of increasing medication and malaria drug resistance, the climate change, and perpetual underinvestment in delicate healthcare systems.
